The art of performing a penetrating keratoplasty (PKP) lies in placing the suture that fixes the donor cornea in such a way that no additional iatrogenic astigmatism is induced, thereby preventing persistent deterioration of visual acuity.
One approach that has proven effective throughout the history of corneal transplantation is fixing the corneal graft with a continuous cross-stitch suture – the double running crossed diagonal suture according to Hoffmann with 2 x 8 passes.
As the name suggests, the challenges of this suturing technique lie in the precision with which the threads must be placed. The technique also requires precise judgment to estimate dimensions, which often comes with a flat learning curve, meaning the desired result is frequently not achieved.
